Installation fails when attempting to create a database instance. (NETIQKB49658)

Situation

Installation fails with error: 'Unable to connect to server Servername using Windows account WindowsUser'.
Installation fails with error: 'Unable to connect to server Servername using SQL account SQLUser'.
The SQL instance where the AMPP database is to be installed is not using the default SQL port (1433).
The installation program cannot connect to server.
The installation program does not prompt for a port number for the SQL instance.

Resolution

The installation program displays a DB Server Info window containing the Database Server field. To set the port number for the SQL instance, add a colon and the port number to the name. For example, SQLServer:1496 will specify the server SQLServer using the port number 1496.

NOTE: Due to the way InstallAnywhere works, if you have made an unsuccessful attempt to connect to the SQL Server during AMPP installation, and you click the 'Back' button to try to re-enter the SQL connection information, the installer does not pick up those new values.  In order to successfully install AMPP, you will need to exit the current installation attempt, and re-run the installer from scratch, using the correct connection information.

Cause

The default port for SQL instances, port 1433, is not being used by the SQL instance.
