cause
Too many items are entered in the 'sqlcqr.sqlcqr_pwdmgr_pending_notify' table.

fix
To resolve this problem truncate the 'sqlcqr.sqlcqr_pwdmgr_pending_notify'  table.

NOTE:  The entries in this table will be lost after the truncate.
