symptom
Error: 'ORA-12640-Authentication adapter initialization failed.' received when starting the 'VSM DB Agent Service' or the 'SQLSecure Notification Agent' service.

cause
This issue has been observed on Oracle databases that have been installed on Microsoft Windows 2000. When logging in as the 'SYSTEM' user (Local System), with startup mode set to Automatic, it is possible that the Oracle database service starts but the database itself does not start automatically.

fix

To correct the error:

  1. Browse to the ORACLE_HOME\network\admin directory.
  2. Open the sqlnet.ora file.
  3. Check the  sqlnet_authentication_services parameter. If it is set to NTS, comment it out or change it to NONE. 
  4. Save the file and close it.

Other services like Oracle Enterprise Management Agent and Oracle Enterprise Manager Management Server, may fail to connect for the same reason; the database has not been started. For more information regarding this error, check www.oracle.com.
