symptom
Mailboxes are not deleted when the policy to 'delete mailboxes when a user is deleted' is enabled.

cause
The cause of this issue is that the User's mailbox is hidden from the GAL.  When a mailbox is hidden, Directory and Resource Administrator (DRA) cannot see the mailbox and therefore will be unable to delete the mailbox when the account is permanently deleted.  If you try to hide a mailbox in DRA, it will produce a warning message stating that this mailbox will no longer be visible and DRA will not be able to manage it.

fix

Although this is designed behavior, here are two possible workaround solutions:

  1. Do not hide the mailbox and let the DRA exchange policy delete the mailbox when the account is deleted.
  2. Create a trigger when the user is permanently deleted to delete the hidden mailbox.
