symptom
The mailbox stores fields are blank, when creating a mailbox for a user.

cause
The issue is caused if the MCS Onepoint Admin Service account is not an Exchange Full Administrator in the Exchange Organization.

fix

The issue is resolved by adding the MCS Onepoint Admin Service account as an Exchange Full Administrator in the Exchange Organization.  This can be performed by the following steps:

1. Launch the Exchange 2000 System Manager tool as a user who has permissions to modify Exchange permissions.
   
2. Right-click the Organization node and select Delegate control...
   
3. On the Exchange Administration Delegation Wizard screen, click Next.
   
4. Click Add...
   
5. On the Delegate Control dialog box, click Browse and select the MCS Onepoint Administration Service Account.
   
6. Click OK.
   
7. In the Role drop down list, select Exchange Full Administrator and click OK.
   
8. Restart the MCS Onepoint Administration Server Service.
