cause
There is an additional space in the Exchange 5.5 Display Name between "US" and ",CN".

For example:

Failed to create CR for cn=Lastname\, Firstname\, Middle\, CO US ,CN=Users,dc=domain,dc=com

fix

Remove the extra space from the source account name.

For example:

Failed to create CR for cn=Lastname\, Firstname\, Middle\, CO US,CN=Users,dc=domain,dc=com
