Unable to unlock account in SSPR

Situation

User account locked in LDAP directory by intruder detect
User cannot log in to SSPR
Error 5069 when authenticating to SSPR
Forgotten password does not show link to unlock

Resolution

Set the following TWO settings in SSPR Configuration Editor under   Modules ⇨ Public ⇨ Forgotten Password ⇨ Profiles ⇨ default ⇨ Options    
Allow Intruder Unlock  (Adds "Unlock" button to Forgotten Password)
Allow Forgotten Password when Locked (Allows access to Forgotten Password when account is locked)

With these settings in place an "unlock" link will show on the Forgotten Password page:
