Resolution
The cause for this issue is broken symbolic links for the Dhost Icons
directory on the 64 bit eDirectory 88 SP3 installs.
To fix this problem, do the following:
- cd /var/opt/novell/eDirectory/data/nds-http
- rm iconsole
- ln -s /opt/novell/eDirectory/lib64/nds-modules/nds-http/iconsole iconsole
-Restart eDirectory and Dhost icons should be visible now.

Additional Information
Note that the above problem has only been reported so far for 88
SP3 64 bit installs.
Other cases have been reported of iMonitor icons disappearing intermittently with eDirectory 88 SP1 and SP2 on Linux and Solaris, but that is a separate issue and is being investigated by NTS currently.
