Unable to see the Failover Device after upgrading to NetWare 6.5 Support Pack 6

Symptoms:
Unable to see the Failover Device after upgrading to NetWare 6.5 Support Pack 6
Command "LIST DEVICES" displays only one instance of the Storage Adapter

Changes:
Down Server Upgrade from NetWare 5.1 Support Pack 8 to NetWare 6.5 Support Pack 6

Corrective Action:
Apply MM.NLM patch dated 8th December 2006 or later

Notes:
For Example, if the first instance of the Storage Adapter is
LOAD QL2x00.HAM SLOT=3 /LUNS /MAXLUNS=8 /ALLPATHS /ALLPORTS /IGNCFG (STARTUP.NCF)
LIST DEVICES shows the following output on System Console

0x18 [V599-A3-D0:1]
0x19 [V599-A3-D1:0]

If the second instance of the Storage Adapter is
LOAD QL2x00.HAM SLOT=4 /LUNS /MAXLUNS=8 /ALLPATHS /ALLPORTS /IGNCFG
(STARTUP.NCF)
LIST DEVICES shows the following output on System Console

0x18 [V599-A4-D1:1]
0x19 [V599-A4-D0:0]

The first instance of Storage Adapater is overwritten by the second instance
